Job Summary
RN Case Manager (Home Health) provides ongoing assessment of the terminal diagnosis impact on patients and families, implements and revises the individualized plan of care, educates patients/families on disease process and end-of-life care, coordinates services with the hospice interdisciplinary team, initiates preventive and rehabilitative nursing procedures, supervises LPNs and paraprofessionals, maintains compliance, participates in training and performance improvement, and rotates on call. Requires at least one year of nursing experience, current Virginia (or compact state) RN license, CPR certification, valid driver’s license, and proficiency with Microsoft Office and Google Suite. Schedule is Full-Time, Mon-Fri 8a-4:30p, with on-call duties as assigned; compensation $42-$45 per hour plus mileage reimbursement.

- Unencumbered Registered Nurse License issued by the state of Virginia (or compact state)
