RN Case Manager (Home Health - Office) FT

Manage patient care and scheduling while maintaining accurate, detailed reports and records. Process, validate, and analyze OASIS data to create 485 forms, then modify treatment plans based on physician orders and patient responses. Coordinate with the health care team to assess, plan, implement, and evaluate care plans, monitoring all aspects of patient care including POC and goals. Instruct families on health education and disease prevention, while ensuring staff are educated on home environment safety risks. Inform physicians of patient conditions as required and observe nurses during visits to ensure proper nursing care. Professionally manage patient, staff, and doctor issues as a liaison between the doctor and staff, directing infection control programs and recommending appropriate treatments. Engage in training or research activities related to nursing as assigned by management.
