Job Summary
As the RN Case Manager for Interim HealthCare in Lexington, MA, oversee the delivery of clinical and caregiver services to ensure high-quality patient/client care in accordance with home health laws and regulatory standards. Work closely with the Director of Health Care Services and Administrator on OASIS admissions, recerts, resumptions, DCs, and overall case management, coordinating with LPNs, CNAs, aides, and other staff to meet patient goals. Enforce Home Health orders from PCPs and coordinate delivery of medical supplies/equipment while training staff in care procedures. Assess patient/client needs, resolve issues, and maintain a high level of satisfaction. Manage financial resources related to the delivery of quality patient/client care. Must be a graduate of an accredited nursing program with an active MA RN license (or BSN), have a minimum of 1 year in home healthcare/OASIS processes, and at least 1 year of supervisory/caregiver-management experience, with CPR certification and strong knowledge of home health laws and regulatory standards.
Required Qualifications
- Graduate of an accredited nursing program
- active RN license in Massachusetts or BSN
- Minimum of 1 year of nursing experience in home healthcare/OASIS admissions, recerts, resumptions and DCs
- At least 1 year of supervisory and caregiver management experience
- CPR certification
- Extensive knowledge of state and federal home health laws and regulatory standards
- Excellent management, organizational, problem-solving, communication and interpersonal skills
