RN Case Manager - Home Health

Conduct in-person patient interviews and comprehensive physical assessments to support home health clients. Oversee the implementation and ongoing assessment of patient care plans by managing home health aides, LPNs, RNs, and other caregivers. Communicate patient conditions and collaborate with providers to deliver evolving care, while providing education on procedures such as wound care, IV administration, and medication management. Work to decrease readmissions by promoting preventative care and ensuring continuity. Requires current unrestricted RN licensure, CPR/AED/BLS/First Aid certification, and one year of home health experience. Full-time role with flexible scheduling in Westmoreland County and East end Pittsburgh.
