Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital or rehab discharge. Examine patients, conduct assessments, document progress, and report changes to physicians. Educate patients on their care plans, medications, therapy, diet, and exercise. Coach family members on patient plans, medication, and home safety. Coordinate discharge from services while ensuring goals are met. Work with a team of physicians and oversee LPNs, CNAs, and aides. Earn $75k-$100k annually with weekly bonuses, health benefits, and flexible assignments. Required Qualifications
- Registered Nurse (RN) with active, unrestricted license in the State of Michigan
- At least one (1) year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
