RN Case Manager - Home Health

Conduct in-person patient interviews and comprehensive physical assessments to initiate care. Oversee the implementation and ongoing assessment of patient plans by managing home health aides, LPNs, RNs, and other caregivers. Communicate patient conditions and collaborate with providers to deliver evolving care needs. Provide education to patients and families on procedures including wound care, IV administration, and medication management. Work to decrease readmissions by promoting preventative care and ensuring continuity. Requires current unrestricted RN license, CPR/AED/BLS/First Aid certification, and one year of home health experience. Full-time role in South Hills with competitive pay ranging from 80K to 95K.
