Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital or rehab discharge. Examine patients, conduct assessments, document progress, and report changes to physicians. Oversee LPNs, CNAs, and aides while educating patients on their care plans, medications, and home safety. Coach family members on patient plans and coordinate discharge services. Ensure goals are met within the Skilled Home Health Services team expanding into the Kalamazoo area.
Required Qualifications
- Registered Nurse (RN) with active, unrestricted license in the State of Michigan
- At least one (1) year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license
- Current car insurance
