RN CASE MANAGER- Home Health

Assess patient and family learning needs, establish realistic goals, and develop individualized plans of care in collaboration with patients, families, and healthcare providers. Manage assigned case loads by following physician orders, reconciling medications, and performing physical assessments while supervising home health aides. Promptly triage patient visits, messages, and phone calls according to priority and urgency. Effectively manage care delivery within the team model, communicate with all disciplines on critical needs or crisis interventions, and complete documentation accurately and timely. Perform on-call responsibilities for assigned patients and families. Participate in agency quality management, safety activities, and ongoing staff meetings.
