Job Summary
Conduct comprehensive physical, social, and psychological assessments to develop holistic care strategies and plans. Coordinate patient care delivery, manage resources, and maintain meticulous patient records while collaborating with interdisciplinary teams. Initiate referrals and actively engage patients and families to ensure personalized solutions. Requires current RN license in Nebraska, Home Health/Hospice experience preferred, and full range of movement for patient transfers. Full-time role in Lincoln, NE with competitive pay of $48 to $50 per hour.
Required Qualifications
- Current Registered Nurse (RN) license in Nebraska
- Valid Driver's License (clean record) and reliable transportation
- Proof of current auto insurance
- Current CPR Certification
- Negative TB Test
- All PCHHC Healthcare employees must be able to pass a criminal background check
Desired Qualifications
- Home Health/Hospice experience as a Registered Nurse preferred
