RN - Case Manager Home Health (Champaign Region)

Home Health RN/Field Case Manager in the Champaign region coordinates and provides direct and indirect patient care within an interdisciplinary team, including LPNs, HHAs, PT/OT/SLP/MSW, across counties such as Champaign, Vermilion, and others. Responsibilities include coordinating care plans, educating patients and families, performing skilled nursing assessments, and managing complex care needs (e.g., wound care with wound vac, ostomy, PICC lines/ports, TPN/antibiotic infusions, catheters/tubes, chest/drain management, and tracheostomy care). Duties also cover completing admissions, OASIS/485 changes within 24 hours, ensuring accurate physician orders, supervising Home Health Aide activities, and collaborating for seamless care transitions. Must demonstrate knowledge of Medicare Home Care benefits, regulatory documentation standards, and ensure timely communications with physicians and the care team. Travel within the geographic area is implied to support patient populations and families.
