RN - Case Manager Home Health (Champaign Region)

Home Health RN/Field Case Manager coordinates and provides direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs across Champaign, Vermilion, Edgar, Iroquois, Ford, Coles, Douglas, Piatt, Moultrie, and Macon counties. Independently and as part of an interdisciplinary home care team (LPN, HHA, PT, PTA, OT, OTA, MSW, SLP) to coordinate care plans, educate patients/family, and perform skilled nursing assessments. Responsibilities include complex wound care (wound vac, ostomy), PICC lines/Ports/Blood Draws, TPN/Antibiotic Infusion, catheters/tubes/drains (PleurX), tracheostomy management, monitoring and educating on health conditions to promote independence. Completes required OASIS/485 changes and admission documentation, ensures timely orders and documentation, supervises Home Health Aide activities, initiates case conferencing, and communicates with multidisciplinary teams and physicians to maintain continuity of care. Demonstrates knowledge of Medicare Home Care benefits, follows regulatory documentation standards, and performs home health case management to meet patient and family needs throughout the geographic area.
