RN Case Manager, Home Health

Provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer ongoing care, develop initial treatment plans based on physician orders, and revise care plans through continuous assessment. Coordinate care across disciplines including physical therapists and home health aides, while supervising their documentation and personal care instructions. Report patient progress to physicians and clinical managers, implement care plans with patient families, and manage discharges with complete clinical documentation.
