Job Summary
RN Case Manager / Recertification Nurse in a Home Health Network, traveling within Southwest & Mid-Central Ohio to coordinate care, perform SOC, admissions, recertifications, and care services. Lead a team of clinicians, ensure documentation and compliance across state/federal guidelines, report to Regional Program Administrator/Director of Nursing, and maintain a weekday 8am–5pm schedule with no mandatory overnight shifts. Salary $78,000–$83,200 annually with potential bonuses; benefits include mileage reimbursement, PTO, and health plans. Requires RN license, nursing degree, OASIS/NON-OASIS experience, CPR/First Aid, and ability to travel up to ~2 hours radius.
Required Qualifications
- Graduate of an accredited School of Nursing
- Active RN License in Ohio
- Minimum of 1 year nursing experience in home health agency or equivalent
- Minimum 1 year of nursing supervisory/leadership experience
- Experience with OASIS and NON-OASIS assessments
- CPR and First Aid Certifications (can be obtained before hire)
