Job Summary
RN Case Manager in Dallas–Fort Worth area providing professional, compassionate home health care for clients in clients’ homes. Responsibilities include managing diverse client needs with Home Health expertise, documenting care using OASIS/Kinnser, and responding to PRN emergency calls. Requires active Texas RN license and Driver’s License with minimum 1 year of Home Health experience; ability to travel locally and work flexible schedules. Office is on the road, visiting client homes throughout Dallas–Fort Worth and surrounding areas.
Required Qualifications
- Graduate of an accredited Nursing Program
- Active Texas RN License
- Driver’s License
- Minimum 1 year of Home Health experience
- Experience with OASIS/Kinnser (preferred)
