Job Summary
PRN RN Case Manager provides comprehensive care management for patients in the Triangle Area, conducting patient assessments, developing and adjusting care plans, educating patients and families, and coordinating care across settings. The role involves field work with regular weekly travel to client homes throughout WAKE County and surrounding areas, supervisory visits every 90 days (60 days if CAP), and collaboration with primary nurses and field supervisors to monitor patient progress. Requirements include an active NC (or multi-state) RN license, nursing program graduation, minimum 2 years of nursing experience, strong communication and advocacy skills, EMR familiarity, and reliable vehicle for travel. Required Qualifications
- Active RN license in North Carolina or multi-state license
- Graduation from an accredited nursing program (as indicated by transcript or diploma)
- Minimum of 2 years of nursing experience (case management experience preferred)
- Ability to read, write, and effectively communicate in English
- Strong clinical skills and understanding of case management principles
- Prior home care experience strongly preferred, but not required
- Own reliable vehicle
- Flexibility
- Ability to travel to cases as assigned
- Experience with electronic medical records (EMR) preferred
