Job Summary
The RN Case Manager will independently visit patients in their home environment, providing comprehensive assessments and care coordination, including skilled nursing tasks and health education. This full-time position features weekday hours with holiday and weekend rotations as needed.
Required Qualifications
- Current RN license in the State of MN
- Graduate of an accredited school of nursing
- CPR Certification
- Reliable transportation and valid driver's license
- Proof of auto insurance
- 2 years of current nursing experience preferred
- 1 year case management experience preferred
Desired Qualifications
- Community health experience
- Wide range of Medical-Surgical experience
