Job Summary
The RN Case Manager in Home Care enjoys independence and flexibility in providing assessments, care coordination, and skilled nursing services directly in patients' homes within a specific geographic area. This role requires strong clinical skills and two years of nursing experience, with a preference for case management experience.
Required Qualifications
- Graduate of an accredited school of nursing
- Current license as a Registered Nurse in the State of MN
- Reliable transportation and valid driver’s license
- CPR Certification
- Two years of current nursing experience preferred
- One year case management experience preferred
Desired Qualifications
- Community health experience
- Wide range of Medical-Surgical experience
