RN Case Manager - Home Care

Conduct head-to-toe assessments, coordinate care, and manage disease and medication plans for patients in their home environments. Perform skilled wound care, catheter management, and ostomy education while preventing infections and ensuring home safety. Visit clients to evaluate care quality, review clinical notes, and participate in interdisciplinary assessments. Counsel patients and families on healthcare needs, complete accurate clinical documentation, and communicate with the healthcare team to coordinate care and plan for discharge.
