RN Case Manager - Home Care, Full-time

Screen, evaluate, and treat patients referred by licensed providers while coordinating treatment activities with the interdisciplinary health care team. Develop care plans in collaboration with physicians, patients, and caregivers upon admission, then periodically review and recertify these plans as appropriate. Provide ongoing clinical assessments of functional and emotional status, document evaluations and objectives, and teach self-care techniques including medication and diet instructions. Offer health counseling to patients and caregivers, provide clinical oversight to LPNs and LNAs, and communicate significant changes to physicians and VNA colleagues. Advise other disciplines of discharges in a timely manner. Requires an unencumbered RN license in Vermont and New Hampshire, a driver's license with a clean record, and one year of home health experience is preferred.
