Job Summary
Conduct initial and ongoing client assessments in the home and develop non-medical care plans based on client needs. Oversee caregivers assigned to cases, provide guidance on client-specific expectations, and ensure services align with approved plans. Monitor client conditions, communicate changes to leadership and families, and document assessments and case notes accurately. Assist with quality assurance visits and maintain compliance with company policies and state home care requirements. This role focuses on assessment and care plan oversight rather than skilled nursing, medication administration, or hands-on clinical care.
Required Qualifications
- Active RN license in good standing
- Reliable transportation
- Valid driver's license
