Job Summary
RN Case Manager - Home Care Assessment Only: focus on in-home client assessments, development and oversight of non-medical care plans, caregiver supervision, and ensuring care aligns with approved plans. No skilled nursing, no medications, no hands-on medical care. Requires active RN license, strong communication/docs, in-home assessment comfort, and reliable transportation. Flexible schedule with meaningful work supporting seniors and families; opportunity to coordinate care while avoiding hands-on clinical tasks.
Required Qualifications
- Active RN license in good standing
- Prior home care, senior care, case management, or assessment experience preferred
- Strong communication and documentation skills
- Comfortable completing in-home assessments
- Ability to build positive relationships with clients, families, caregivers, and office staff
- Reliable transportation and valid driver’s license
- Compassionate, dependable, and detail-oriented
