RN Case Manager- Home care assessment only

Conduct comprehensive in-home patient assessments to determine care needs and eligibility, evaluating health status, functional abilities, and home environment safety. Develop and document individualized care plans based on assessment findings, ensuring accurate and timely clinical documentation in compliance with regulatory standards. Collaborate with physicians, care teams, and family members to coordinate appropriate services, monitor patient conditions, and adjust care plans as needed. Serve as a patient advocate by connecting clients with community resources and support services. Requires an active Florida RN license, experience in home health or case management, strong clinical assessment skills, and proficiency with electronic health records.
