Job Summary
RN CASE MANAGER responsible for discharge planning and proactive utilization management to optimize patient outcomes and reimbursement. Collaborates with patients, families, and the healthcare team to ensure safe, individualized discharge planning and effective post-discharge resources. Requires nursing licensure in NV, 3+ years clinical experience, and knowledge of utilization management and post-discharge community resources; strong computer skills needed.
Required Qualifications
- Graduate of an accredited school of nursing
- Current license to practice as a Registered Nurse in the State of Nevada
- Minimum three years experience in varied clinical settings
- Two years experience in Utilization Review, Utilization Management or Case Management preferred
- Knowledge of social and physical factors affecting discharge
- Knowledge of community resources to meet post-discharge needs
- Computer proficiency (word processing, spreadsheets, data collection/management)
