Job Summary
RN Case Manager responsible for discharge planning and utilization management to maximize reimbursement for patient visits. Collaborates with patients, families, and healthcare team to ensure safe, individualized discharge planning and timely utilization management. Requires Nevada RN license, 3+ years clinical experience, and 2+ years in utilization management or case management; strong computer skills and knowledge of community resources for post-discharge needs.
Required Qualifications
- Graduate of an accredited school of nursing.
- Minimum three years experience in varied clinical settings.
- Two years experience in Utilization Review, Utilization Management or Case Management preferred.
- Knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
- Current license to practice as a Registered Nurse in the State of Nevada.
