Job Summary
RN Case Manager at Henderson Hospital responsible for discharge planning and timely utilization management to optimize patient outcomes and reimbursement. Requires nursing education, an active Nevada RN license, and a minimum of three years of clinical experience (with preferred utilization review/management or case management exposure). Must understand social and physical factors affecting discharge and identify post-discharge community resources. Proficient in computer-based data collection/management and standard office software. Collaboration with patients, families, and the healthcare team is essential to ensure safe, individualized discharge planning.
Required Qualifications
- Graduate of an accredited school of nursing
- Current license to practice as a Registered Nurse in the State of Nevada
- Minimum three years experience in varied clinical settings; two years in Utilization Review/Management or Case Management preferred
- Knowledge of social and physical factors affecting discharge; knowledge of community resources for post-discharge needs
- Computer proficiency including word processing, spreadsheet, and data management programs
