Job Summary
RN Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The RN visits patients in their home, including personal residences, assisted living, skilled nursing, and other settings. Responsibilities include comprehensive assessments, developing and implementing a plan of care, coordinating patient care for an average 12-15 patient caseload, administering prescribed medications and treatments, counseling patients and families, and facilitating activities of daily living and self-sufficiency. The wage is described as dependent on hospice experience; suggested salary range is $40-$54.79/hour.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state of Washington
- Graduate of an accredited school of nursing
- Maintains a current BLS certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
- Ability to participate in the backup on-call rotation on occasion
- A minimum of two years' experience, at least one of which is in public health, home care, or hospice nursing, is preferred
