RN Case Manager

Conduct initial and ongoing comprehensive assessments of terminal disease impacts on patients' physical, psychosocial, and environmental needs. Implement individualized plans of care, recommending revisions as necessary, and collaborate with patients, caregivers, physicians, and healthcare professionals. Attend inter-disciplinary group meetings to coordinate care planning, prioritize needs, and promote care coordination. Perform on-call responsibilities and submit all documentation timely while ensuring compliance with local, state, federal laws, Medicare regulations, and personnel policies.
