RN Case Manager

Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs. Implement individualized plans of care and recommend revisions. Collaborate with patients, caregivers, physicians, and healthcare professionals. Attend Inter-Disciplinary Group meetings to coordinate patient care planning and prioritize needs. Perform on-call responsibilities to provide service to patients and families as assigned. Submit all documentation timely and ensure compliance with local, state, and federal laws.
