Job Summary
Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs. Implement individualized plans of care, recommend revisions, and collaborate with physicians, caregivers, and interdisciplinary groups to coordinate patient care planning. Perform on-call responsibilities to provide service to patients and families as assigned. Ensure timely documentation submission and overall compliance with local, state, federal laws, and Medicare regulations. Submit applications with a current driver's license, reliable transportation, and auto liability insurance. Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
- reliable transportation
- provide ongoing valid and current auto liability insurance
- Two years of experience as an RN

- preferred in a healthcare/clinical setting
