RN Case Manager

Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs. Implement individualized plans of care, recommend revisions, and collaborate with physicians, caregivers, and interdisciplinary groups to coordinate patient care planning. Perform on-call responsibilities to provide service to patients and families as assigned. Ensure timely documentation submission and overall compliance with local, state, federal laws, and Medicare regulations. Submit applications with a current driver's license, reliable transportation, and auto liability insurance.
