Job Summary
Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs, including ADLs and grief risks. Implement individualized plans of care, recommend revisions, and collaborate with patients, caregivers, physicians, and healthcare professionals. Attend Inter-Disciplinary Group meetings to coordinate the patient care planning process and prioritize needs. Perform on-call responsibilities to provide service to patients and families as assigned. Submit all documentation timely and ensure compliance with local, state, federal laws, Medicare regulations, and personnel policies. This role requires a current State Registered Nurse License, two years of RN experience preferred, and a valid driver's license with auto liability insurance. Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
