Job Summary
Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs, including ADLs and grief risks. Implement individualized plans of care and recommend revisions as necessary. Collaborate with patients, caregivers, physicians, and healthcare professionals to coordinate care. Attend Inter-Disciplinary Group meetings to participate in patient care planning, prioritize needs, and promote coordination. Perform on-call responsibilities to provide service to patients and families as assigned. Submit all documentation timely and ensure compliance with local, state, federal laws, Medicare regulations, and personnel policies. Requires accredited nursing degree, current RN license, and two years of clinical experience preferred.
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
