Job Summary
Conduct initial and ongoing comprehensive assessments of terminal disease impacts on patients' physical, psychosocial, and environmental needs. Implement individualized plans of care, recommending revisions as necessary, and collaborate with patients, caregivers, physicians, and healthcare professionals. Attend inter-disciplinary group meetings to coordinate care planning, prioritize needs, and promote care coordination. Perform on-call responsibilities and submit all documentation timely while ensuring compliance with local, state, federal laws, Medicare regulations, and personnel policies. This full-time role involves travel and offers competitive pay with medical, dental, vision, PTO, 401k matching, tuition reimbursement, and a company car for qualifying individuals.
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
- reliable transportation
- provide ongoing valid and current auto liability insurance
- Two years of experience as an RN

- preferred in a healthcare/clinical setting
