RN Case Manager

Assess, plan, coordinate, monitor, and evaluate services required to respond to individual patient needs under the supervision of the Director of Continuum of Care Department. Manage utilization review, discharge planning, and care coordination with the patient, family, physician, and health care team. Collaborate with other Continuum of Care Department members to meet patient needs and plan, implement, and evaluate care plans before, during, and after procedures to optimize outcomes. Requires an RN with at least one year of acute clinical experience and two years in managed care or hospital case management.
