Job Summary
As an RN Case Manager at Wesley Medical Center, coordinate services, develop individualized care plans, and guide patients and families through program orientation, discharge planning, and education to support treatment planning. Collaborate with physicians, nurses, and the interdisciplinary team to ensure safe transitions and patient-centered care, documenting findings and communicating needs across the care team while promoting compassion, respect, and excellence.
Required Qualifications
- Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse (RN)
- Current Basic Life Support (BLS) verification through American Heart Assoc. and American Red Cross - Healthcare Provider CPR
- 3 years of experience in a clinical setting hospital nursing experience.
