Job Summary
Conduct comprehensive evaluations and continuous assessments of patient needs while providing direct patient care in both home and facility settings. Develop, implement, and revise care plans as needed to ensure the highest quality of end-of-life care, including pain management and wound care. Collaborate effectively with healthcare professionals to guarantee continuity of care and engage actively in interdisciplinary meetings. Be a strong advocate for patients and their families, ensuring their voices are honored. Educate on Advance Directives and oversee home health aides with assurance. Requires current RN state licensure, reliable transportation, and strong scheduling capabilities.
Required Qualifications
