RN Case Manager

Coordinate patient care and family communication for hospice assignments in Enid, initiating contact with physicians and agencies to optimize resource use. Conduct regular patient visits, manage admissions and deaths, and maintain up-to-date records reflecting physical, psychological, and spiritual factors. Instruct caregivers on medication administration, personal care, and end-of-life preparation while providing support at the time of death. Share in 24-hour, seven days a week coverage and attend team meetings and care plan conferences. Evaluate hospice aides biweekly and LPNs monthly.
