Job Summary
Receive patient assignments and initiate communication with attending physicians and hospice staff to coordinate optimal care. Cultivate communication and bereavement skills while maintaining regular contact with families regarding new assignments and ongoing needs. Obtain data on physical, psychological, social, and spiritual factors to incorporate into the Plan of Care and maintain up-to-date patient records reflecting changes as they occur. Instruct patients and caregivers on medication administration, personal care, mobility, and preparation for end-of-life events. Provide appropriate support at the time of death and share in 24-hour, seven days a week coverage. Evaluate hospice aides biweekly and LPNs monthly. Attend team meetings and care plan conferences as required.
Required Qualifications
- Registered Nurse currently licensed in the state in which the care is provided
- Previous experience in hospice, home health, geriatrics or general medical/surgical nursing
- Demonstrated commitment to the hospice philosophy of care
