Job Summary
Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients while minimizing fragmentation of the healthcare delivery system. Provide care coordination between patients, families, physicians, providers, the community, and Regional Operations Managers. Assess patient needs, develop and manage individualized care plans, and support patients through complex healthcare transitions. Coordinate services across interdisciplinary teams and demonstrate effectiveness in a change agent role. Manage clinical, financial, and patient satisfaction outcomes using CQI techniques. This role requires a Registered Nurse with case management experience or equivalent experience in assessing patient needs and managing care plans. The position is full-time, Monday through Friday, with a salary range of $81,000 - $99,000 per year.
Required Qualifications
- A.D.N degree from accredited school of nursing
- Current Registered Nurse (RN) license in the state of practice
- Continuing education credits maintained as required by state of practice
- Minimum of five (5) years' experience in clinical nursing
- Case management experience
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM)
- Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
- Demonstrated experience and effectiveness in change agent role
- Demonstrated knowledge and understanding of CQI techniques
- Previous experience in healthcare performance coaching
- Current CPR certification
- Ability to modify personal practice patterns to adapt to new / electronic processes and increased productivity expectations as it pertains to Capella implementation
- Current driver's license in state employed with positive driving record and able to meet requirements of insurance coverage
- Basic computer skills and proficiency in MS Word and Outlook
- Functional proficiency with DaVita specific clinical software programs, including Capella
- Home office with internet connectivity at minimum of 1MB upload and 1MB download speed
- Three-year diploma from accredited diploma program may be substituted for nursing degree
- Minimum of three (3) years' experience in renal nursing
Desired Qualifications
- B.S.N preferred
- Minimum of five (5) years' experience in clinical nursing is strongly preferred
- Minimum of three (3) years' experience in renal nursing preferred
