RN Case Manager- Full Time - Hospice

Job Summary
RN Case Manager for Hospice coordinates admissions and nursing care for patients in their place of residence, collaborating with the Interdisciplinary Group/Team, patient/family, and referring agency. Responsibilities include conducting Nursing Assessments to identify physical, psycho-social, and environmental needs; developing and documenting the care plan; providing nursing care with infection control; coordinating with the Interdisciplinary Group/Team to meet patient/family needs; communicating with attending and hospice physicians and other care professionals; timely paperwork submission; participation in in-service and QAPI programs; promoting Hospice philosophy and policies; meeting productivity standards; performing on-call duties; ensuring effective communication with patients, families, and care teams; maintaining transportation and driving requirements; and traveling to patient homes and office/support locations as needed. Works in patient homes with exposure to varied conditions; requires flexible scheduling, ability to travel locally, and ability to lift up to 50 lbs.
Required Qualifications
- Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current State license as a Registered Nurse; Current driver’s license
- Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills
