Job Summary
Provide nursing care as an RN Case Manager for Hospice patients, applying hospice philosophy and interdisciplinary collaboration to develop and implement individualized care plans. Responsibilities include evaluating outcomes using clinical and utilization data, defining patient/family care units, guiding the plan of care with the team, applying age-specific care methodologies, explaining advanced directives, ensuring appropriate documentation, and facilitating group education when needed. Requires independence, critical thinking, and the ability to coordinate admissions and ongoing resident care within a 60‐mile radius of Nisswa, MN. This is a full‐time day-shift role with on-call weekend rotation and one weeknight per week, with 32 hours per week scheduled.
Required Qualifications
- Bachelor's degree in Nursing strongly preferred
- Current RN license in state of practice or multi-state license (NLC)
- Minimum of two years clinical experience preferred
- Valid driver's license
- Understanding of hospice/palliative care principles
- Ability to document clearly and communicate with interdisciplinary team
- Knowledge of reimbursement for services
- Ability to lead and educate staff
- Age-specific care methodologies knowledge (neonatal to geriatric)
- Interdisciplinary planning and care coordination
Desired Qualifications
