Job Summary
RN Case Manager in Home Health setting. Serve as the primary clinical partner for patients and families, delivering one-on-one nursing care in the home, conducting comprehensive in-home assessments, providing skilled nursing interventions (wound care, infusions, catheter care, medication management, post-operative and chronic disease management), educating and empowering patients and caregivers, monitoring progress, coordinating care across providers, and completing timely documentation to support quality outcomes. Schedule is Monday–Friday with weekend on-call rotation; territory includes Cedarburg, Glendale, Bayside, and surrounding communities. Requires active RN license, 1+ year of nursing experience, valid driver’s license, CPR certification (or ability to obtain). Benefits available for full-time roles.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least 1 year of clinical nursing experience
- Current driver’s license, reliable transportation, and auto insurance
- CPR certification (or able to obtain certification)
