Job Summary
Assess hospice appropriateness, gather patient data, and complete admission paperwork via EMR. Counsel families during interviews to determine care levels and physical/psychosocial needs. Place orders for medications, durable medical equipment, and oxygen as required. Consult with hospital, skilled nursing, and assisted living staff to facilitate discharge to home or facility. Communicate pertinent information to the interdisciplinary team to ensure a smooth transition into hospice care. Requires Colorado RN licensure, BSN preferred, and 1-2 years hospice or acute care experience. Full-time day shift role with a sign-on bonus available.
Required Qualifications
- Graduate of an accredited school of nursing
- BSN
- 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health
- Current Colorado RN licensure
- Valid Colorado State driver's license
- Auto insurance
- CPR certification
- Ability to lift minimum of 35 lbs
- Minimum Education: Graduate of an accredited school of nursing; BSN preferred
- Minimum Experience: 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health
- Required License: Current Colorado RN licensure
- Valid Colorado State driver's license and auto insurance
- CPR certification required
- Ability to lift minimum of 35 lbs
