Job Summary
RN Case Manager in Fairfax, VA with Bristol Hospice. Provide individualized, whole-person hospice care; develop and implement care plans based on medical history; deliver one-on-one support to patients and families; assess physical, emotional, and spiritual needs; monitor vital signs; administer physician-ordered medications; educate families on care techniques and symptom management; document changes and collaborate with interdisciplinary teams to ensure comprehensive care and quality care delivery; travel to patient locations as needed; CPR required; RN license in good standing and valid driver’s license with reliable transportation. Required Qualifications
- A valid Registered Nurse license in the state you will work in
