Job Summary
RN Case Manager responsibilities include managing discomfort and providing symptom relief under physician direction in palliative/end-of-life care; perform initial and ongoing assessments of patient needs across physical, functional, psychosocial, and environmental domains; collaborate with the Interdisciplinary Group (IDG) to develop, implement, and update individualized care plans, coordinate preventive and rehabilitative procedures, and initiate referrals as needed; educate patients, families, and caregivers about disease processes, palliative interventions, and safety practices.
Required Qualifications
- Graduate of approved nursing program and current unrestricted Florida RN license.
- Valid and current CPR certification required.
- Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
