Job Summary
Provide expertise and leadership to ensure effective resource management for patient care delivery across the care continuum. Collaborate with licensed practitioners and inter-professional team members to coordinate care, plan transitional care, and facilitate discharge needs. Anticipate patient educational requirements and expedite early discharge planning by initiating links with post-acute external resources. Utilize the nursing process to deliver individualized care, including triage, assessment, and referrals to appropriate teams. Advocate for patients across various care settings while enhancing clinical outcomes and patient satisfaction. Requires a Bachelor's degree in Nursing (preferred in most markets) and unencumbered RN license.
Required Qualifications
- Bachelor's degree in Nursing
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
- Incumbent must obtain and subsequently maintain required department specific competencies and certifications including Basic Life Support (BLS) within 60 days of hire
Desired Qualifications
- Bachelor's degree in Nursing strongly preferred in Bemidji, Wisconsin, Michigan and Rapid City
- Minimum of two years' clinical experience preferred
- Certification in: Case Management (CCM), or holds a certification in their patient population specialty that they serve, is encouraged when eligible
