Job Summary
As an RN Case Manager, you will provide high-quality nursing care to patients in their homes, including assessments, complex nursing treatments, and education aimed at avoiding readmissions. Responsibilities include performing complete admission assessments, coordinating patient care across disciplines, and managing pain documentation and prioritization. This role demands a commitment to excellence as part of the Cleveland Clinic team, including handling documentation and care coordination remotely after patient visits. The position requires a Bachelor’s of Science in Nursing (BSN) as a preferred qualification, while mandatory qualifications include a current RN license, BLS Certification, and relevant work experience.
Required Qualifications
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
- Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
- One year of current experience as an RN
- One year of experience in an acute Medical/Surgical or Home Care setting

- Bachelor’s of Science in Nursing (BSN)
Additional Requirements
- Testing for controlled substances upon offer of employment
- Compliance with an influenza prevention program, including obtaining an influenza vaccination or obtaining an approved exemption
