RN Case Manager

Assess, plan, implement, and evaluate end-of-life care needs for patients in home or community settings while directing and supervising hospice aide staff. Develop patient care plans addressing identified problems and goals during on-call hours, weekends, and holidays. Admit eligible patients in collaboration with the triage team and attend interdisciplinary meetings for full team discussion. Maintain care coordination notes, arrange medication and supply delivery after business hours, and report patient status to clinical leadership and physicians. Requires current unrestricted RN licensure, two years of RN experience, and two years of hospice/palliative nursing experience preferred. Schedule includes Mon-Fri 8am-5pm with on-call rotation.
