Job Summary
Assess, plan, implement, and evaluate end-of-life care needs for patients in home or community settings during on-call hours, weekends, and holidays. Direct, supervise, and instruct hospice aide staff in personal patient care while admitting eligible patients in collaboration with the triage team. Develop patient care plans addressing identified problems and goals, ensuring adherence to the 4Ms framework and Age-Friendly Care standards. Maintain care coordination notes, report patient status to clinical leadership and attending physicians, and assist with medication and supply delivery outside standard business hours. Report on-call activities to clinical leadership and Branch RN Case Managers at the end of each shift. Attend interdisciplinary team meetings during on-call periods for full team discussion and evaluation of patient needs.
Required Qualifications
- Current unrestricted RN licensure in the state of practice
- Valid driver's license and proof of auto insurance
- Two years of RN experience
Desired Qualifications
- Two years of Hospice/Palliative Nursing experience preferred
