Job Summary
Provides end-of-life hospice care in patients' home or community settings, administering intermittent skilled nursing services, directing and supervising hospice aides, and coordinating care across the care team. Responsibilities include managing an on-call patient/care plan, coordinating medications and supplies after hours, participating in interdisciplinary team meetings, admitting patients to hospice under physician order, communicating patient status with clinical leadership and physicians, and applying the 4Ms (What Matters, Medications, Mentation, Mobility) to deliver Age-Friendly Care. Requires strong clinical judgment, compassion, and effective communication to support patients and families during highly sensitive experiences.
Required Qualifications
- Current unrestricted RN licensure in the state of practice
- Valid driver’s license and proof of auto insurance
- Two years of RN experience
- Two years of Hospice/Palliative Nursing experience preferred
