Job Summary
Vitra Health seeks Temporary Nurse Case Managers (RN) to provide in-person regional coverage on a contract basis. The role conducts in-person intake, nursing assessments, care planning, coordination of care, caregiver education, and ongoing monitoring to support elders or adults with disabilities while maintaining safety in home/community settings. Responsibilities include conducting assessments and reassessments, developing/adjusting plans of care with the interdisciplinary team, medication reviews, discharge planning, and ensuring compliance with program requirements. Clinician will educate clients and caregivers on health maintenance, disease prevention, medication management, and access to community resources; perform regular in-home visits; document accurately per organizational/state requirements; and travel within MA to various regions. Requires MA RN license, 2+ years direct care experience with older adults or disabilities, valid driver’s license, reliable transportation, and strong communication and documentation skills. Travel, in-home visits, and use of company tablets are expected; equal opportunity employer.
Required Qualifications
- Current Massachusetts nursing license (RN) in good standing
- Minimum of two (2) years of recent experience providing direct care to elders or adults with disabilities
- Valid driver's license, reliable transportation, and proof of insurance
- Willingness and ability to travel throughout assigned regions
- Strong communication, interpersonal, documentation, and organizational skills
- Ability to work independently while effectively managing multiple priorities
- Strong problem-solving and critical-thinking abilities
